About CDSMP

The Chronic Disease Self-Management Program (CDSMP) is a workshop for adults with at least one chronic health condition, which may include epilepsy. It focuses on disease management skills, including decision-making, problem-solving, and action planning.

What is the benefit?

This interactive program aims to increase:

  • Confidence
  • Physical and psychological well-being
  • Knowledge of ways to manage chronic conditions
  • Motivation to manage challenges associated with chronic diseases.
What do I need to participate?

To participate in the program, you will need to register online.   We will provide you with a “Living a Health Life with Chronic Conditions” program book. You must also be available to participate in the 6 weeks of sessions to complete the program. All participants will be asked to complete surveys before starting the program and then again after completing the program to help evaluate the program.

How does it work?

Participants attend an interactive workshop once a week for six weeks to learn problem-solving, decision-making, and other techniques for managing problems common to people with chronic diseases. These workshops are offered in person, virtually, or over the phone. In a typical workshop, participants set a realistic goal for the upcoming week and develop an action plan for meeting that goal. They report on their progress at the following workshop and ask for feedback from the group to help address any challenges.
